About the Badge of Dishonour
In December 2024, Chief Coroner HHJ Alexia Durran introduced an unprecedented accountability mechanism: a public list of organisations that had failed to respond to Prevention of Future Death reports within the statutory 56-day deadline.
The list is published every six months and was the first formal enforcement tool for a duty that has otherwise carried no penalty since it was established in 2013.
